Arrival at the Office

On the day of your appointment, please be sure to arrive at least 15 minutes early to fill out or turn in any paperwork or insurance information. You may fill out these forms before you arrive at the office.

Bring these items to your first appointment:
  • Photo ID
  • Insurance information
  • Patient history form
  • Appropriate copay, coinsurance and deductibles information
  • List of current medications


Reviewing Your History

One of our medical assistants will review your patient history with you. He or she may ask additional questions to help create a complete picture of your treatment history and current pain.

Diagnosis

After this review, one of our orthopedic physicians, physician assistants or nurse practitioners will assess your condition. We spend time with each of our patients in order to fully understand the cause of his or her pain.

Many times, our physicians can make a diagnosis based on patient history, physical examination or x-ray. Our offices at Little York Road and Yankee Street are equipped with diagnostic imaging equipment, which allows us to get immediate results. If further imaging is needed, we usually can perform tests before you leave our office.

Treatment

Once the physician has determined the cause of your pain, he will work with you to create a treatment plan tailored to your needs. If your treatment plan includes further testing, surgery and/or physical therapy, we will schedule those appointments for you as well.
